Treatment of Virus-negative Acute Myocarditis

Acute virus-negative lymphocytic myocarditis iv IgG ?
Giant Cell Myocarditis Ciclosporine: drugs trough 150-200µg (week 1.-4.)
drugs trough  100-120µg (from week 5.)
Prednisolone: 1mg/kg BW (for 2 weeks)
dose reduction: 10mg every 2 weeks
maintenance dose : 10-15mg/day
In fulminant cases: Antithymocytic Immunoglobulin (ATG)
3-4mg/kg BW for 12h/day for 5 days
Sarcoidosis Prednisolone: 1-1.5 mg/kg BW (4weeks)
gradually dose reduction: 10mg every 2 weeks
maintenance dose: 10-15mg/day for 6 month
Eosinophilic Myocarditis Specific treatment differs significantly based on the underlying aethiology

Prednisolone: the initial dosage and treatment duration vary significantly with respect the severity and the underlying disorder
1mg/kg BW (for at least 4 weeks until clinical response is reached)
dose reduction: 10mg every 2 weeks
maintenance dose : 10mg/day
duration of therapy: 6-9 month

in case of relapse: Prednisolone 10mg/day continuously

Combined immunosuppressive therapy comprising corticosteroids and
cyclophosphamide or azathioprine are usually administered in a more
advanced state

Humanized monoclonal AB Mepolizumab which inhibits the binding of
IL-5 to its receptor and is administered in CSS or HES

Treatment of Virus-negative chronic Myocarditis / Inflammatory Cardiomyopathy

Azathioprine: 50mg/12h for 6 month
Methylprednisolone: initial dose 1mg/kg/day after 4 weeks: decrease
by 10mg then after every
3 weeks 10mg maintenance dose: 5-10 mg/day

Treatment of Virus-associated chronic Myocarditis / DCMi

Enterovirus, Adenovirus Interferon-β: 4-8 Mio IU s.c. every 2. day (for 24 weeks)
ciHHV6-A/B Gancyclovir: iv therapy: 5mg/kg BW i.v., 2x/12h for 5 days
Valgancyclovir: 900mg (1800mg) /day p.o. 8 Mio IU s.c. every 2. day
(week 3-24)
In fulminant cases: Antithymocytic Immunoglobulin (ATG)
3-4mg/kg BW for 12h/day for 5 days
Symptomatic reactivated ciHHV6 Valgancyclovir: 900mg (1800mg) / day p.o. for 6 month
Parvovirus B19 (mRNA negative)
+ inflammation
